I am a UK tax payer and would like the Huntington’s Disease Association to treat all donations I have made for the past four years prior to this year and all donations I make in the future as Gift Aid donations, until I notify you otherwise.
I confirm I have paid or will pay an amount of Income Tax and/or Capital Gains Tax in each tax year that is at least equal to the tax that the Charities and CASCs I donate to will reclaim on my gifts.
I understand that other taxes such as VAT and Council Tax do not qualify and that the Huntington’s Disease Association will reclaim 25p of tax on every £1 that I give.
